Our Charge/Payment Policies
Cosmetic Surgeries: Payment for all cosmetic procedures (non-insurance related) is due one week prior to surgery and can be paid by cash, credit card (accept Debit, Visa and MasterCard) or cashier’s check. (No personal checks please)
ANESTHESIA PAYMENTS: The same policy applies to the anesthesiologist and collected at the facility. Cash, Credit Card, or cashier’s check. The Doctor’s Professional Billing Service for anesthesia can be contacted at (503) 297-7223.
Items that are not included in the facility fees are implants, pain pumps, durable medical items (braces), and laboratory and pathology services. In the event that this service is required, you will receive a separate bill from each company providing these services. Typically these services are generally discussed before surgery.
Insurance Procedures:
Our charges consist of one fee for each procedure performed. This fee covers Operating Room use (facility fee), supplies, dressings, medications and nursing services. Items that are not included in procedure fees are: Special implants and orthopedic implants to name a few. These items will appear as a separate line item on our bill.
Upon your request, your estimated insurance coverage and amount of your co-pay will be available before surgery. These fees will be estimated, based on the diagnosis and scheduled procedures received from your physician. We will bill your insurance for you and attempt to assist you in resolving insurance payment issues. You should contact your insurance carrier for other specific concerns. We gladly accept Debit, Visa, and MasterCard for patient’s portion of fees. IF you paid in advance, any credits due will be refunded after insurance payments are received.
CHARGES THAT ARE NOT COVERED IN OUR BILL…
1.) Anesthesia Services; 2.) Surgeon’s charges; 3.) Laboratory Fee; 4.) Durable Medical Equipment (DME). You will receive a separate bill from each company providing these services. We will be able to assist you with contact information, if you are uncertain about where to call for clarification on these bills.
